This form must be submitted by a representative of the mare's boarding farm prior to arriving at the Breeding Shed. It is the responsibility of the mare owner or his agent to present the proper mare for breeding. The manager of the Stallion may refuse breeding if, in his/her sole opinion, it might be detrimental to the health or safety of the personnel, the Stallion or the Mare. The Stallion owner, Taylor Made Stallions and their respective agents and employees are not responsible for and are released and indemnified by mare owner from and against damages for any accident, injury or disease incurred by the Mare or resulting in the death of the Mare. All mares must have proper identification when presented for breeding (halter or neck strap with her name on it). IF THE MARE DOES NOT HAVE PROPER IDENTIFICATION AND ALL REQUIRED PAPERWORK, SHE WILL NOT BE BRED. PLEASE BE ON TIME.
